To select a DEPARTURE, ARRIVAL, CHARTER, or TRANSFER form click on the appropriate word above.  The form you request will be loaded and filled in with the original information that you completed your original registration form.  To get this form you will be asked to LOGON using your USER NAME and PASSWORD.  Once the form loads just complete the missing information, look over the completed information and make any necessary changes.  Then submit the request.  It is as simple as that.  Have fun.
